Is this new pain cause by Chiari?

I have constant pain from chiari and I am used to having extreme pressure on the left side of my head (if I had to explain the pain, I would say it feels like an ax is stuck in the back of my head at all times and when I laugh or cough it feels like someone kicking me in the head as hard as they can.

But now I am getting a sharp, electric shock type of pain on the right side of my head. At this time, I am only experiencing this when I laugh and feels different compared to the pain i’m used to.

I have not had an MRI or CT since 2018, was told I needed decompression surgery but that I would need to lose weight before and I haven’t got there yet. Could this be a new symptom of my Chiari?
