Monthly Chiari Pain Diary

Doctors are always asking me to keep a pain diary to rate my pain. When I am hurting I don't feel like writing paragraphs. Sometimes there are several symptoms that overshadow other ones. And when I'm feeling better, I can't rememer all the things that were wrong.

I created a chiari pain template that you can print out each month to track your symptoms.

Now the docs will see how this thing affects us.

There is a phone app called My Pain Diary that I use. You can rate your pain level, add notes, create graphs, even send things directly to a doctor. It's been great! I have an iphone, and don't know if its available on other types of phones, but if so, I highly recommend it!

I know how you feel, Nykki. My Neurology PA wanted me to keep a daily headache diary after my first visit with her, and it didn't get very far at all. I managed to log 2 days in it, then the 3rd day I went out to run errands, had a bad headache hit me at Walmart, and couldn't remember all the details of that day... after that, nothing got written down. I've never been very good about keeping any sort of diary or journal anyway, and with my headache issues lately, I've been even more forgetful than usual. Might take a look at your template though, whenever I get Microsoft Office on this computer.
