Home Injection Medications

I ended up back in the ER (Sacred Heart) today with a really severe migraine. The nurse came in did his thing and so did the ER doctor. They called my neurologist (nothing new there) and my neurologist suggested having a combination of the medications that they give me in the ER as an inject able cocktail that I can "check out" from his office on a one vial basis and return the empty ones with an effectiveness report for him to keep in my records. From what he said, it would be a kit with the medication AND the syringes that would be needed to do the injections and it would all be returned before I could check out another one. He even brought up changing the pain management agreement so that I could do the injections under the observation of an RN at the local hospital if needed.

The cocktail they are talking about is Decadron, Dilotted, DHE, Benadryl and promethazine pre mixed for either 1 or 2 injections as well as training my other half on how to give them. He said it was completely up to me but last time I checked, Dilotted could only be used in a clinical setting.

My big question is should I go for it or tell my neurologist that I feel more comfortable in having the injections done in the ER or office.

Here are some details that Beeba pointed out that I hadn't included before. I have tried all triptan medications, my body has become resistant to hydro-codone and oxycoton. Vicodin is also out of the question because of a resistance problem with it. Fiorcet is also out of the question because last time I took it I ended up having uncontrolled bleeding issues and frequent bloody noses that would go for hours. I am currently in the final steps before surgery, should be having it done in July or August of this year if everything keeps going. The doctor is also someone I trust, he did a lumbar decompression, lamanectomy, and fusion on my mom back in 1995 and she hasn't had any problems with her lower back since; he has also now reached out to Chiari procedures.

The medication cocktail is only going to be available as a last resort; meaning I have to try all other options including over the counter and oral rescue meds that I already have. It would also have a max of 2 doses a month and they have to be at least 10 days apart before he'll authorize them being given.

Does anyone have any ideas that I could bring up to my neurologist since he is now also working as my pain management doctor when it comes to my migraines? I could really use some suggestions.

Thanks in advance!

Hi...

Sounds like your doc is willing to help you, for sure...I have personal experience with the cocktail injection you mentioned..however, minus the Dulaudid (sp)

One time at the ER for a severe h/x the doc gave me a combo of Decadron and Benadyl..just those two...I am seeing now that you mentioned DHE and promethazine as well.

I had good results with the 2 meds I mentioned.I am throwing this out to you ...Wondering if your doc has ever used the 2 in combination I mentioned. I, personally think it is worth a try..also...neither drugs are controlled...so the doc may be more apt to let you try it at home without having to go back to his office with the empty vial, ect...just a thought.

How are you feeling now?? keep me posted.

As a nurse, my concern for you is that you have become SO desensitized to so many narcotics, when you have surgery-- there is nothing else left to help your pain. I have worked with several different pain management groups, and it is sad to see post op surgery patients suffering because narcotics do not work for them. Just be careful. Dilaudid is a strong drug and you may need it when you have surgery. I hate to see you become desensitized to it also.

Hope you can get theHMO to clear you for surgery if it’s what you’re ready to do.I’ve just had decommpression and I am glad I did it,the only regret I have is not being able to get it sooner. You will know when the time comes if you are ready,it is a hard thing to do.Hopefully you can get to the right specialist to do the surgery, you may find it less painful than what you’re facing now, sounds as if you’re getting to a place of less options.Hope you soon find the right answer,be thinking of you.
